If you are looking to apply to our school please use the button on our home page that says “Apply Now.” You will be asked to create a FACTS account and then go through an application process. If your application is approved by the school you will be notified to go through our enrollment process. Both the application and enrollment process is done online through your FACTS account. Below are forms that need to be filled out for admission into the school once you have been accepted as a student.
Please review the attached documents to determine what you will need to provide for your child for the 2023-2024 school year.
Physical: K or 1st ( if did not provide in K) and 6th grade ( 7th graders who did not return in 6th grade) PHYSICAL_EXAM_form_
Immunization record: K and new enrolls and 7th graders (required Tdap and MCV vaccines) PA_Immunizations
Dental: K or 1st (if did not provide in K), 3rd and 7th grade SSD_Dental_Exam_Letter-updated
Asthma/Allergy/ Medication Administration Form: Only if your child has a diagnosis do these forms need to be returned.
Emergency Cards: Will be sent home the first week of school, please fill out the front and back on the two copies.
